Depends which country you're in
Many R rated movies in US are classified as MA15+ in Australia (MA = Mature Audiences). Playing with her pussy would attract an R or X rating in Australia, while full frontal will only get MA or MA15+ rating.

In the US, R rating is for any nudity beyond very brief stuff. Any sex is fully simulated. It's for normal scripted movies, not porn. You don't even usually see vulva, just hair.

Porn level stuff would be NC-17 at bare minimum if it's unsimulated sex in a proper mainstream film (rare), or possibly not rated. But actual porn tends not to be released in movie theaters anymore, which is what she was doing on OF.
