With DiGen I describe everything medically. Shaft, glans, scrotum, foreskin, Urethral Opening, testicales...Use terms that cause it to draw from clinical knowledge and you'll get more anatomical results. Doesn't always work though. It's annoying, so if you get something that works, save it in a notes doc.

It's because of the way the AI was trained. By default most big AI models don't include NSFW images so it can be used by everyone. It's the smart pervs among us that expand the original AI model and add the NSFW data in later. Sometimes these hero's, princes among men, can make a mistake here and there. Leaving you with a weird looking dick But new training data is usually updated to the AI and you will see better results over time.

Maybe:
Try a different prompt - As Rockula said in his post, use the medical terms to describe body parts.
Try using a picture with a different angle/view point - The AI will use a different, hopefully better reference to create your image/video.
Or just try a different AI, one a bit better suited for NSFW, there are loads popping up online.
